Serpentine belt replacement belt came off again after it was replaced?

I was replacing the serpentine belt after it broke, and the belt came off again after it was replaced. The nut on the crankshaft fell off and was lost. What type of nut do I need to buy to replace the one that was lost? Also why did the belt come off after it was replaced? What is the easiest way to replace the belt?

The belt falls off because the belt tensioner bearing goes bad or you have broken accessory brackets. Sight the belt alignment through it routing and see if you can see which pulley is off. Also is the belt making noise while the engine is running. Can you please shoot a quick video with your phone so I can hear the noise? That way I can tell for sure what's going on.
