head gasket replacement in june 2008 (including full head skim), no oil in water, or water in oil, using too much coolant, having to top up weekly full amount of water/coolant. Also radiator fan does not appear to be working. Could this be contributing to the problem? Thanks for your help.

you need to discover why the fans are not working ? start by checking the fuse .. also check there are no coolant leaks !
