Stalls out when driving and jerks when stopping

My car stalls out when driving it and when I switch gear it jerks and it jerks when I'm coming to a stop.

When it stall out can you hear the fuel pump running in the tank? When it wont run turn the key to the on position to see if you can hear the pump running for five seconds in the tank.
