Transmission problem

1. Car often takes a long time to shift up but will always eventually do so.
2. Recently, after it stops at the traffic lights, nothing happens when you depress the accelerator to go, then the gear would suddenly engage with a judder. Once this starts, the "juddering" will increase in frequency. This occurs only after the car has warmed up and will behave normally again after the engine cools down.
