Reduce power light and check engine light comes on

Both lights come on and I have to pull over. I shut the car off wait two minutes and start it again lights are gone, but returns five minutes later. The car also jerks when you keep it a constant speed. I Cleaned the throttle body and replaced the valve cover gasket. But still have the same problem.
