Rear heater core delete procedure

How do I do a rear heater core delete on this van? The plumbing is rusted out. I did cut out the bad area and use 5/8 hose to repair a couple years ago, but it is leaking again. I am done trying to fix these rusted pipes. Can I just run a hose from the engine outlet pipe to the heater core, then from the heater core back to the thermostat? Do I need to use a "T" in there somewhere?

You have right idea, but do the disconnect and coupling at engine. see picture connect 5 and 6 together or cap off.

Thank you for the reply. Looking at the picture, Couldn't I just connect 3 to 4, then 2 to 6?

I did this and it worked great! No more dealing with those rusted out lines for the rear heat!???????? Super easy to do. Just loosen up the power steering reservoir and the coil pack and move aside then you'll have plenty of room to get the job done. I hope this idea helps someone else
