Hi,
When you adjust the spindle nut, do you have the caliper already mounted? If you do, remove the caliper and then adjust the nut. The caliper can be holding the rotor at an angle and when you drive, it moves, and then there is play in the bearings. It's a theory at this point, but I've seen it happen before. There really isn't much more that could be causing it unless you aren't reinstalling the cotter pin to prevent the nut from turning.
I attached the directions for replacing the disc rotor. Take a look through them and let me know if it appears you are missing anything. Also, let me know if you are hearing any sounds when this happens.
