Radiator fan sometimes works

My van runs good, two weeks ago it starts running hot. Replaced relays, sensor, and radiator fan. Gauge inside says hot. What could be problem? Why would the fan do that?

The fans do that when the head gasket is blown. Lets run this test to see what happens. Here is a guide to help us get started:
