If you are purchasing an aftermarket re manufactured pump and it keeps happening, chances are it is nothing more than a bad pump. However, when you replace the pump, you should flush the old fluid from the rack and pinion as well as the lines to prevent dirt from re-entering the new pump. And yes, I have seen issues where the hoses have deteriorated internally and caused pump problems.
If you have been using the same brand each time, try a different brand and make sure to flush the system before refilling and bleeding the system. Here is a link which discusses power steering problems:
