When I start the engine the first rev is perfect.
When I keep it over 4,000 rpm's it start to lose power until under 4,000 rpm's.
When idle for about three to five minutes and you rev it up the first rev is good there after it just go bad again.

Welcome to 2CarPros.

This could be a couple different things. What I suggest is to first check fuel pressure. If it is low, it will cause a power loss when there is a load on the engine or the RPM's are high. If the pressure is good, then I want you to check the catalytic converters to make sure they aren't partially plugged.
Here are two links. One discusses how to check fuel pressure and the other how to check a catalytic converter.

Let me know what you find or if you have other questions.

Take care,
Joe
