There are many causes that make problem as you mentioned, to narrow down the problem as a first step check the fault codes. check figure 1 below location of scan tool connector to you car.

You can visually check for any vacuum leak that can cause these problems, jumped out throttle air intake boot or idle control problem. check the figure 2 below for vacuum diagram that can help.
