what causes a head gasket to blow on a vehicle that has never required mechanical repairs other than maintenance?

getting them hot can do it.
but other than that, things just happen.
they can just fail.
be glad you dont have a dodge with a 2.4. they pretty much failed before they got to 45,000 miles.

Overheating is probably the #1 cause, but head bolts can break, or pull out of block because of a bad casting.
