1995 Plymouth Voyager serpintine belt problems

1995 Plymouth Voyager 6 cyl Front Wheel Drive 90000 miles

My serpintine belt keeps coming off. The belt is only a couple months old and is in good condition. I have checked the pullies and belt for oil or other fluids and they are all clean. The pullies all seem to turn free when the belt is off. It seems to stay on for about 1-1 1/2 miles before it comes off. Can you give me any ideas as to why this happens?

If it's coming off every mile or so, I imagine you're putting it back on yourself and thus could perhaps tell whether the tensioner is able to keep it taut. If the tensioner is worn out or broken, that might be the cause, assuming none of the pulleys are misaligned, bent or loose and the belt is put on correctly.
