1995 Plymouth Voyager Heater Barely Works

My Heater doesn't work on my 95 Plymouth Voyager. Even with the engine at full running temperature, the air comes out only lukewarm and that doesn't even last that long. I have been told that my heater coil could be failing, the blower fan doesn't work, and that my heater core could be clogged. Which is the best possibility and how do I fix it?

Clogged heater core is a possibility. First I'd check to make sure the thermostat isn't stuck open.
