1995 Plymouth Voyager emissions failed....

Can you folks decipher this emissions report results
(limit/actual reading):

IDLE (800rpm)
HCppm........ 200/90 pass
CO%.......... 1.00/1.02 fail
NOppm....... n/a n/a

FAST (1300rpm)
HCppm...... 54/56 fail
CO%........ .30/.04 pass
NOppm...... 757/256 pass

It was so close to passing. Seafoam treated, new plug wires, 6mos old plugs, diaelectric grease, new air filter, oil change 10w-30, gas antifreeze w/91 octane, shrouded pipe leading to cat, new pcv valve. Did I miss something? Based on the results, what could it be? Help!
