I think your idle speed is too low.

Would that cause it to only run ruff in drive and not in neutral?

Yes it will. You may notice the difference in your engine speed at "neutral" and in "drive". Also, while you are checking your idle speed, check the idle air control valve. This valve is supposed to automatically correct the idle but apparently it is not doing its job.
