91 Acclaim Trouble Code 35

My 91 Plymouth Acclaim 4 Cyl that has about 250 k miles is having trouble code 35. I have found that is "Cooling Fan Relay Circuit Open or Shorted". Now, I need to know how to fix it. Any advice/direction. Thanks.

The cooling fan relay( left inner fender). Dk blue wire= hot with ignition switch, gray= hot all the time from battery, lt green goes out to fan and turns into either blk/white or solid black, dk blue/pink is the signal from pcm to turn fan on. Check for powers at relay to start
