Overheating problem

What would cause water to boil back out of the reservoir tank? I took term. Completely out and it still doing it. What are the possible problems?
Also, where is fuse location for fan?

Fuse and relay for fan should be under hood and marked on under side of fuse box cover. If fan is not coming on check those. Then when hot disconnect fan and check radiator for cold spots if any cold spots radiator is no good. Then fill coolant system and pressure check for a head gasket problem auto parts rent tester. Also available is chemical tester either is okay to use.
