Hello, I wanted to know if it is possible I could reach the outer lines of the heater core that is running all the way down to the passenger side of the van underneath?

Good morning,

Yes, they are a pain to replace. You need to remove some thing to get to them especially when installing the new ones. The replacements comes as two in a set and are a challenge to set correctly. They were installed originally without the engine in the van.
Very common failure.

Good morning Mr Roy.

Thanks for your reply and you were so helpful yes it is a pain so therefore I have decided to cut off the two places that was damaged and going to replace it with two 58" rubber hoses with four clamps two on each side of the hose. So I am asking now, what are your suggestions on my decision? Would that work?

Yes, that will work but it is temporary. The areas you attach it to are most likely rotted inside as well. Keep and eye on it for future leaks. It was a lousy design.
