Oil in the coolant car getting hot fast

We recently changed our radiatir and radiator hose on our 2004 Chrysler Pacifica because we found oil sludge in the radiatir and in the coolant. Come to find out oil leaks out fast and is going into the coolant what could be causing this to happen?
