Code P0134, O2 sensor

My car is showing (no activity detected bank 1 ). what does this mean? I have change my sensor twice.

I attached the testing for this code. Basically it is saying that the sensor is installed wrong or you have a connection issue at the PCM.

Run through this testing and see what turns up. Thanks
