2004 Nissan Sentra difficult to shift

my sentra has taken a lot of wear and tear and has a very difficult time shifting. first gear is tough but reverse is the hardest. i am a 200 pound man and i need both hands and a lot of strengh to put it in reverse. is this the clutch or the transmission? and if either is it a job i can do myself with a chiltons or hanes manuels?

with the car off can you move the shifter freely into the gears but its hard or grinds when running? or is the shifter just real stiff and hard to move?
