I have a 1988 Nissan Sentra hatchback with a 5-speed transmission. Recently, it began to pop out of fifth gear if you started to go up even a small hill without downshifting. Then, it feels like it is harder to find fourth gear when shifting from third and it is more difficult to find reverse when shifting from neutral after the engine has started. And, sometimes I seem to need to depress the clutch a second time in order to start the car. Lastly, there is a grinding noise which disappears shortly after depressing the clutch and returns when the clutch is released. What could be going on?

Sounds like you need a new clutch assembly.
