2005 Nissan Maxima Noise at start and during acceleration

2005 Nissan Maxima 6 cyl Wheel Drive Type unknown Automatic 53,000 miles

My car makes a squeaky belt noise at start and stops once car is shifted into gear. The car also makes a loud squealing noise during acceleration around 40mph. I have all maintainence performed on time and all major work performed by the dealship.

When was the belt replaced last? Has the tensioner been inspected to b esure it is holding adequate tension on th ebelt? Does it do it with a/c on or off or does the a/c not matter?
