1991 Nissan 240SX Acceleration & Grayish smoke

I have a 91 Nissan 240sx, manual transmission. There is a couple of things I'm having problems with. The first is that while I'm driving and i push the gas pedal to the floor, my rpm's go way up, but i don't accelerate as fast as i should. Another thing is that, if I'm driving for a while, and I come to a stop, as soon as i accelerate, grayish smoke comes out of my muffler. Any advise would be great. thanks.

Sounds like the clutch is slipping could mean free play and height adjustment if not time for a new clutch assembly. For the smoke could mean rich mixture-
