Yep, the mode actuator will cause this issue, here are the instructions to change it out with the location of the actuator in the images below.

MODE DOOR MOTOR: Removal and Installation
REMOVAL
1. Remove heater & cooling unit assembly..
2. Remove mounting screws, and then remove mode door motor.
3. Disconnect mode door motor connector.
INSTALLATION
Note the following items, install in the reverse order of removal.
CAUTION:
- Replace O-ring with new ones. Then apply compressor oil to them when installing.
- Female-side piping connection is thin and easy to deform. Slowly insert the male-side piping straight in axial direction.
- Insert piping securely until a clicks is heard.
- After piping connection is completed, pull male-side piping by hand to make sure that connection does not come loose.
- Check for leakages when recharging refrigerant. See: Refrigerant > Procedures > Leak Test.
NOTE:
See: Coolant > Procedures > Refilling (VQ37VHR) or See: Coolant > Procedures > Refilling (VK56VD) when filling radiator with engine coolant.
