May be a misfiring injector, if all electrical concerns are okay, check injector pulse with a noid light, this will show that the electrical impulse is there to fire the injector if that is okay, remove the injectors and have them ultrasonically cleaned and flow tested. If the fuel system is very dirty, particles can bypass a clogged filter and lodge in an injector causing problems. But first double check all what has been done first to make sure you have not created a problem unknowingly. Also, best do a compression test to prove cylinder pressures are okay.

I have added a few links to assist you.
