The milky substance probably just has not made it to the cap just yet, but the oil pan would be the first place it accumulates, but it would usually mean the coolant is getting into the oil pan and could be coming in from a blown head gasket and a head gasket can blow in between the oil and coolant spots and not affect the cylinder at all. So there would be no white smoke out the tailpipe. I doubt if the head is cracked unless of course the damage did get pushed in enough to reach the engine.
