1992 Mercury Sable gear broke

What causes the gear on bottom of distributor to break off ? It is the second time in 2000 miles.

There is also a gear on the end of the camshaft that drives the distributor gear. This gear could have become damaged or loose. Also, check for binding of the oil pump or shaft. This is not a normal failure, and there has to be something causing it.
