1999 Mercury Mystique Engine shaking and Brakes

My engine constantly shakes like it wants to die, it is also loud (the exhaust). When I apply the brakes it comes to a slow stop, it doesn't start braking right away and when I am completely stopped, the engine pushes forward like it wants to keep going forward. What is going on?

Is your brake paddle hard to push down
check for vacuum leak around the brake booster
also sound like engine running at higher rpm than normal
