2002 Mazda Tribute Oxygen Sensor trouble

I have replaced the same O2 sensor twice and now the same PO136 code is coming up again. What are the possible causes losing the same sensor in a short amount of time?

that would be a code for the sensor after the convertor. its not really a big problem, basically if the new one didnt help chances are the catalytic convertor is not functinoning properly. or you have an exhaust leak.
