1995 Lincoln Continental lost #2 #4 pistons

what would cause these pistons to be scattered, torn up completely.

Hi there,

Are these side by side, or opposing pistons? was the engine over revved, was there any thing odd before they failed? noise? vibration? had any work been done recently?
