1995 Lexus ES 300 temperature running hot

for the past 2-4 weeks, i have been hearing a humming noise. starting yesterday, after a click, my car ran hot with steam coming from under the hood. i replaced it with coolant and after driving 5 miles, it ran hot again (all of the coolant was gone), with no steam this time, just the temperature gauge (still driving about 5 miles each time). i again replaced the coolant and turned the air conditioner off while turning the heater on (thinking this would help blow hot air from the engine). this time it did not run hot. of note, i replaced the radiator in december. do you think it's the radiator cap? or a hole in the hose? or a clogged radiator. thanks so much!

If its not leaking anywhere try replacing the thermostat and go from here.
