Intermittently codes P0171, P0133 and P2195 appear?

I have engine light on with VSC trac, when I plug obd I get p0171 and p0133 but that's sometimes not all the time or I just get p0171. Now rarely I'll get p0171, p0133, and p2195 sometimes. This all happens when car is driven for a bit and idling is when sometimes lights will go on. I replace new Denso MAF sensor and oxygen sensor upstream, spark plugs and hose coming from manifold purge valve maybe. I test MAF sensor it's good too. I searched for days for vacuum leaks and can't seem to find any.

An exhaust leak will cause this issue as well, does the engine have one?

P0171 - System Too Lean (Bank 1)
P0133 - Oxygen Sensor Circuit Slow Response (Bank 1, Sensor 1)
P2195 - O2 Sensor Signal Stuck Lean (Bank 1, Sensor 1)

If not, I would replace the MAF sensor, cleaning it will not help at this point. Here is the correct way to do the job. Clear the codes once the job is completed. Check out the images (below). Please let us know what happens.

I bought new MAF sensor already and replace and it is Denso. I just now did a smoke test, and smoke came out from the plastic piece that goes in the air intake right before the air filter it has a hose attached to it too. Also, smoke is coming out from the intake manifold, before this I replace intake gasket I don't know if it's not sitting right or what.

Any place smoke is coming out is a leak. This will be the problem; I would recheck the gasket for the intake and repair or replace the air intake boot/tube.
