Car shuttering on take off once at around 15 miles an hour it goes away and shifts fine, now a loud clunk has started and it sounds like in the rear end, it has gotten worse in the last couple days any ideals if it's in the rear end or transmission ???

The shuttering and clunking noise are most likely related and source would most probably be from the U joints of the propeller shafts. Get them checked for wear and excessive clearance.
