These could be a couple of things but let's start by checking the fuel pressure while this is happening because that is the most likely cause of hesitations.

If the fuel pressure is okay, then we could have a boost issue. However, here is a guide that will cover the other basic things that can cause this:

If these things are not causing any issues, then we need to check for codes and see if there is anything going on with the turbo.

If there are no codes, then we need to monitor the manifold pressure with a scan tool and see if there is any movement when this is happening with steady throttle.
This will show us if we have any possible leaks in the charged air side of the system. Basically, that means if you have boost escaping before it gets into the engine then you are going to get surging.
