Jercking and acceleration hesitation, exhaust whistle and inability to climb hill

When I start it after some time it will jerck and if I tries to accelerate while in motion it equally jerck. Very slow in climbing hill and exhaust whistles when I accelerates harder.

Is the check engine light on? Have you checked to see if the catalytic converter is plugging?
