There is a rattle; the source of which I cannot locate; occuring when the Jeep is started. Once I get going, it doesn't want to shift out of 2nd gear. If I let up on the gas it will eventually shift into 3rd gear.

I'm reluctant to dump a great deal of money into this problem especially when the age and number of miles on the Jeep are considered.

Any thoughts on a solution short of rebuilt/replacing the transmission?

Have a look at the tailshaft area, the tranny mout is located there, push up on the tailshft, if it moves, the mount needs to be changed. also check the shifter cable on the drivers side for any missing parts.
