clutch release bearing keeps popping out of pressure plate. what am i missing?

Is wedge collar in place on cover?

yes the wedge collar is in place.it keeps popping back out.I can feel it snap in good.bad wedge collar? Thanks doc>>>

Tea they get worn if it can't hold bearing then replacement could be the fix
