2004 Hyundai Elantra Spark PLug flew out

I had a tune up at 50k. At 70k a spark plug exploded out of my 3rd cylinder. The mechanic is staying this is due to an improper installation at the time of the tune up. I am wondering if this is possible, that a car runs for 20k (plus or minus) after the spark plugs are installed poorly without any other issues and then a catastrophic failure occurs?

If the plug wasn't tight enough, this could happen. Did it come loose or strip the threads from the cylender head?
