having an issue with my mode door actuator?

I was having an issue with my mode door actuator. It would change mode but when it came to putting on any vent mode it will just blow on the passenger side.

I replaced it and it was working for about 4 months and then started doing the same thing. I took a part to look at it and it worked find work for a few weeks and then boom again it happened.

It sounds like the driver's side mode actuator is weak, can I ask which brand the replacement part is? Here is how to replace the driver's mode door actuator correctly and to make sure we are on the same page of its location. Check out the images (below). Please let us know what you find.
