How do I replace the rear wheel bearing?

Loud noise with acceleration wobbling from rear driver side. I don't know the train reactions with bad rear wheel bearings, I need to know what the problems could be and what caused it and if the rear end itself has to be replaced as well and how to change the rear wheel bearing?
