1998 Honda Civic Rough

I have been experiencing a rough idle only when the car is in gear and idle, like when sitting at a red light. I have changed everything, plugs, wires, distributor cap, rotor, etc. The rough idle stops when I either put the car in park or when I put it in neutral. It is annoying and shakes the steering wheel and everything. What could be the problem?

At 150k I would suggest a good look at the motor mounts. When they wear out, they sag, causing the engine to no longer be insulated from the car, thus the vibration you feel
