I have a 1994 Honda Accord EX with 227,343 miles. I just recently had the brake cable, starter and master cylinder replaced. After the starter and mc replacement my D4 dash light started blinking. The car seems to be running fine except that the starter STILL makes a grinding sound when I turn the car over. The mechanic said he thought I needed a tune-up or perhaps the flywheel needs to be replaced too. Should I continue to drive the car with the D4 light flashing? Could this be related to the work I had done? Since the grinding noise is still there did the starter really need to be replaced?

The D4 blinking is just a transmission code. This is not because of the starter or any previous work done to the vehicle.

I personally would get it diagnosed asap, transmission problems are not fun.
