1988 Honda Accord Car cuts out/jerks when warmed up

I have a 88 accord that drives perfectly when you first start it and it's cold. After about 10-15 minutes it begins to jerk, like you're driving too slow in a higher gear. It only does it when it's warm and going under 45 mph. I read online that it was the egr ports, but my boyfriend says thats wrong.
