headlights stay on when switch is off. I replaced switch but still stays on.

Check the relays in the junction box. Should have a high beam and low beam relay. I would disconnect the switch and see if the lights go out. leave it disconnected while you check the relays. The headlights are controlled by the BCM. Its not uncommon for these vehicles for it to fail leave the headlights on.

Yes, you can only get remanufactured from the dealer that need programmed.
