This is typical of a pilot bearing that has gone out, the clutch will need to be removed. Please visit the guide to see what you are in for
Clutch Disc was stuck to the pressure plate. Possibly from riding the clutch too long. We recommended replacing the disc, pressure place and hydraulic release bearing (bearing & output cylinder)
