I've been reading hear about the issue of the engine miss on the Safari And Astro Van. I have replaced a number of caps (GM) and have yet to solve the problem. I also just replaced the distributor with a new unit, not rebuilt and still have the same problem. Next was the coil with no luck. Still trying to figure this out. Thought I would share my exsperiences on this mystery.

Hve you doen the plugs and wires yet? at 138k they are past due.If so those had problems with injectors and fuel pressure regulators causing hard starting and random misfires. Is the check engine light on? If so have it scanned it may lead you to the cylinder that is having the problem
