1992 GMC Safari Serpentine belt won't stay on

Engine Mechanical problem
1992 GMC Safari 6 cyl 350000 miles

During repairs (fuel filter) mechanic also replaced belt as it had some cracking. The new belt would not stay on. He then replaced the tensioner and still didn't help, he tried putting old belt/old tensioner back on, didn't help. He then put new tensioner and old belt on and this worked for a 4 hour road trip (luckily). Since then its been a struggle to try and keep it on for any length of time... 15 min even.

Is there a way to figure out whats causing the problem?

Is the belt routing accurate? All was good till belt was replaced?
