Rough Acceleration after a ride

If I go out and start my car up right now and take it for a ride it will run good for about 5 minutes and it gradually gets worse and worse. Whenever I try to accelerate it just bogs down and jerks a lot if I give it really any more than a little bit of gas. It is the LO3 engine. TBI 305. It can also have a slightly rough idle at operating temprature. Its normally lower rpm also normally under 2000 rpm. I cant give it enough gas to get over 2000rpm. Its just odd because when I get in and start it up it will run fine and then starting at higher rpms it will jerk and bog until the point where it gets to the low rpms and I cant really do anything but accelerate really slow. Ive also replaced the fuel filter too. Any ideas before I take it to a shop.
